Incentives

Shaw offers the following incentives that you can complete and earn additional money into your HSA Bank Account (Associates who are not enrolled in the Shaw Medical Plan or who do not have an HSA Bank Account will receive any earned incentives through alternate fundings as taxable income).  Read below to find out more or for a quick glance, see the 2017 Wellness Incentives summary.

Associate Health Assessment / Biometric Incentive:  $150
Who is eligible?   Full time non-probationary associates
 
What do I have to do?  
Step 1:  See your physician, go to QuadMed, or use the new Quest Diagnostic option to complete your Biometrics.    (If Biometrics are completed by your physician, use the Biometric Screening Form and fax to QuadMed to ensure the results are recorded.)
 
 
Step 2:  Complete the QuadMed Health Assessment located in ShawAndMe OR via www.myquadmedical.com/shaw.
 
Note:   Incentives must be completed by November 30, 2017.  The Associate Health Assessment/Biometric Incentive is the “gatekeeper” incentive. It must be completed before any other incentives will be applied.


Outcomes-Based Goals Incentive OR Reasonable Alternative:  $100
Who is eligible?   Full time non-probationary associates
 
What do I have to do?  
Check the Keas portal after your Biometrics have been submitted to see if you have met the       outcomes-based goals. (If your first Biometric values do not meet the requirements, the reasonable alternative will be provided inside Keas.)
 

Spouse Health Assessment / Biometric Incentive:  $150
Who is eligible?   Spouses who are covered under the Shaw Medical Plan
 

What do I have to do?  
Step 1:  See your physician, go to QuadMed, or use the new Quest Diagnostic option to complete your Biometrics. (If Biometrics are completed by your physician, use the Biometric Screening Form and fax to QuadMed to ensure results are recorded.)

Step 2:  Complete the QuadMed Health Assessment located on the QuadMed website (www.myquadmedical.com/shaw).
 
Important notes:   Biometrics must be completed in 2017.  If you and your spouse both work for Shaw, you would have the opportunity to each earn a total of $250 for the Health Assessment/Biometric and Outcomes-Based Goals Incentive/Reasonable Alternative.
 

Keas Incentives:  $200
Who is eligible?   Full time non-probationary associates
 
What do I have to do?  

REQUIREMENTS FOR COMPLETION:
Keas Mission #1—Complete the Keas Mission between March 6 - May 7 for $100
Keas Mission #2—Complete the Keas Mission between August 7 - Oct. 8 for $100
